Dr. Jih has been awarded a NIH K23 grant for 5 years from the National Institute on Minority Health and Health Disparities for our project, "Addressing Contextual Factors in Chronic Disease Management in Minority Older Adults through Photo-based Patient-Clinician Communication." The project will focus on developing a photo-based communication tool to support discussions related to social determinants of health in chronic disease care among older adults with diabetes and multiple chronic conditions. This photo-based intervention is based on the premise that a photo is A) more than a thousand words and B) can promote efficient information exchange and activate patients to communicate their complex lived experiences that may of which be difficult to elicit or challenging to discuss.
